Ha! If this wasn’t a deliberate reference in Sherlock, perhaps it should have been. The passage that inspires Billy the innkeeper’s words “Sorry we couldn’t do a double room for you boys” comes from The Valley of Fear:

We slept in a double-bedded room, which was the best that the little country inn could do for us. I was already asleep when I was partly awakened by his entrance.

“Well, Holmes,” I murmured, “have you found anything out?”

He stood beside me in silence, his candle in his hand. Then the tall, lean figure inclined towards me. “I say, Watson,” he whispered, “would you be afraid to sleep in the same room with a lunatic, a man with softening of the brain, an idiot whose mind has lost its grip?”

“Not in the least,” I answered in astonishment.

“Ah, that’s lucky,” he said, and not another word would he utter that night. 

And of course, a large part of John McMurdo’s backstory in The Valley of Fear nominally takes place in Pennsylvania. In particular, although Doyle’s Vermissa Valley characters Ettie Shafter and her father were Swedish-American in the published novel, Doyle wrote them to be Pennsylvania Dutch (these are German-American settlers who include the Amish) – paralleling his use of the Mormon community in A Study in Scarlet. He was persuaded by his Strand publisher to change their ethnicity because of the war (this was in 1914).

The town of Intercourse, PA changed its name from Cross Keys in 1815. In the present day, it is known for being the Amish town where the film Witness was shot [x]. So… it’s not inconceivable that this was a deliberate choice.

I had to find out where this really was because looking at it, I felt like I knew exactly where it was. It turns out it’s in Breezewood, PA, and i have never been there, which only serves to highlight the OP’s point.
